This certificate provides two adults with a four night Cruise on a Carnival Cruise from Port Canaveral, FL, or Los Angeles, CA (Los Angeles Cruises depart from Long Beach, CA). The port that you depart from is your choice. The Port Canaveral Cruise includes stops in Freeport and Nassau. The Los Angeles Cruise includes stops in Catalina and Ensenada. The accommodations included in this four night Cruise are in category 1 interior staterooms featuring bunk bed sleeping. To take advantage of this offer, please read and follow the instructions carefully. If you wish to Cruise in better than category 1 accommodations (including staterooms with non-bunk bed accommodations), extend your cruise, move your cruise to a weekend, add additional nights to your Cruise or add additional persons, such changes to your Cruise may be available at an additional cost to you, starting at $130.00 per person. Such changes are solely at your option.

Recipients of a cruise certificate must validate the Registration Form on the certificate by completing it and mailing it to Holiday Travel of America within 60 days from date of issue or this certificate will be void. For proper validation your Registration Form must be issued, dated, and signed by an authorized sponsor (Gold Rush Poker). No facsimiles or photocopies will be accepted; original documents only. Non-California - Details of Participation

  1. You will receive an acknowledgement letter within 14 days of our receipt of the Registration Form. That acknowledgement letter will request your preferred Cruise dates, reservation fee and your preferred departure port. If you don’t receive this letter within 14 days please contact customer service at cs@htoa.com to prevent this offer from becoming void. Please select two sets of dates in chronological order, at least 21 days between choices. Submit these dates, and your preferred departure port, at least 60 days in advance of your earliest departure date. We will seek to fulfill your dates based on availability in chronological order. These dates must be for a Monday departure from Los Angeles, CA, and for a Sunday departure from Port Canaveral, FL (unless you choose to alter your Cruise options as described above at your expense). At the time you send in your preferred dates of travel, you will be required to submit a $25 per person reservation fee and a tax deposit to be used for your estimated government taxes and fees ("tax deposit"). The approximate tax deposit is $134.43 per person (or $268.86 for two people) for Los Angeles departures, and $158.82 per person ($317.64 for two people) for Port Canaveral departures. To complete the validation process, you must submit your tax deposit and reservation fee within 30 days from the date of that acknowledgment letter or your certificate will be void. After your travel date is confirmed, you will receive a confirmation letter, which will disclose the amount of the actual cost of taxes. At that time, any variation between the actual government imposed taxes and your tax deposit will be refunded, or be requested as the case may be.
  2. This certificate does not include airline tickets, airport transfers, ground transportation, taxes, government imposed security fees, miscellaneous personal and incidental expenses, alcoholic beverages, health spa, gratuities, and shore excursions.
  3. All Cruise reservations are subject to space availability, and are restricted, in respect to the specified Cruises, dates of travel, and stateroom category. This offer is not available for travel 5 days before and 5 days after a major holiday (New Years Day, President’s Day, Easter, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas). Space availability may be affected by special events. Once we secure a confirmation you will be contacted approximately 60 days prior to your confirmed travel date.   4. This offer is not valid for groups (anyone attempting to travel with different certificates), and cannot be used in conjunction with any other Cruise offer. It is non-transferable, cannot be sold, has no cash value, for promotional use only and void where prohibited by law. This offer is void if received in the State of California. Due to the promotional nature of this offer, HTOA, Inc. can only honor one certificate, per household, per family, per year. Same passenger cannot redeem a separate certificate within a year. This offer is not available to employees, or friends or relatives of employees of the sponsoring company Reservations are accepted on an individual basis only.
  5. Cancellation & Change Policy: Any confirmed reservation that is changed or cancelled is subject to the following Cruise Line policy: Cancellations of confirmed reservations from the Cruise Line prior to 61 days from departure date is complimentary, 60 to 30 days prior to the departure date is $300.00, 29 to 8 days prior to departure is an additional $250.00 and 7 days or less from departure, there will be no changes and this offer is void. All cancellation/change fees incurred will need to be paid prior to change of a confirmed reservation. Any cancellation without notice will result in this offer being void. Cancellation fees are not being charged to utilize this certificate, but rather are fees incurred to cancel your confirmed reservations. Please keep this policy in mind when you make your reservation selections.
  6. HTOA, Inc. is not to be held responsible for any act of God, and/or any other circumstances beyond our control, causing dates, and/or Cruises to be unavailable. In the event that such a circumstance occurs, we will offer substitute dates and/or Cruises.
  7. Passenger(s) traveling on this offer must be listed on the original Registration Form, and must be at least 21 years of age. If one passenger is under the age of 21, then the other passenger must be at least 25 years of age. Timely validated certificates expire one year from the date of issue. All travel must be completed by that date.
  8. It is important that you do not make travel arrangements or leave for your Cruise until you have received your confirmation.
  9. HTOA, Inc. is not responsible for any verbal or written representation, by any person or company, which is not contained in this certificate.
  10. The company reserves the right to re-instate the fuel supplement for all guests at up to $9 per person per day if the NYMEX oil price exceeds $70 per barrel.
